A bed bug addendum (sometimes called a bed bug disclosure or bed bug rider) is a separate attachment to your residential lease that specifically addresses the history of bed bugs in the unit or building, inspection rights, reporting procedures, and who pays for treatment. While not every state mandates a specific form, California Civil Code § 1954.603 and several other states now require written disclosure of known bed bug infestations in the past 12–24 months.
In my experience representing both landlords and tenants, the absence of a signed bed bug addendum is the #1 reason judges rule against landlords when tenants withhold rent or break leases over infestations. Having tenants acknowledge in writing that the unit was bed-bug-free (or disclosed) at move-in dramatically reduces liability.
California remains one of the strictest states. Under California Civil Code § 1954.603 (effective January 1, 2017, and updated through 2025), landlords must provide every new tenant a specific bed bug disclosure notice. The exact language is provided by the California Department of Consumer Affairs and is available on their official website.

Arizona takes a slightly different approach. Under A.R.S. § 33-1319, landlords must disclose known bed bug history for the previous 120 days and provide educational material. While Arizona does not mandate the exact wording like California, the Arizona Residential Landlord and Tenant Act requires the disclosure be in a separate document.
In my Arizona practice, I recommend using a combined disclosure + addendum that satisfies both disclosure and ongoing responsibilities.
