As a business law attorney who has drafted and reviewed hundreds of employee handbooks for medical practices across the United States over the past 12 years, I know exactly how expensive it can be to hire outside counsel for this single document – often $3,000–$8,000 for a small-to-medium practice. That’s why I created this comprehensive, up-to-date medical office employee handbook template and am giving it away completely free.
This 2025-compliant medical office employee handbook template is specifically written for private medical, dental, chiropractic, optometry, and mental health practices in all 50 states. It incorporates the latest federal requirements (FLSA, FMLA, ADA, OSHA, HIPAA) plus the most common state-specific pitfalls I see practices overlook. You can download the editable Microsoft Word and PDF versions at the bottom of this page – no email required.
In my experience representing over 200 healthcare employers, the practices that face the largest Department of Labor fines, EEOC charges, or wrongful termination lawsuits almost always lack a clear, written employee handbook – or they’re still using a generic internet template from 2018 that never mentions HIPAA privacy officers, OSHA bloodborne pathogens, or the 2024–2025 changes to overtime and paid sick leave laws.
A properly drafted medical office employee handbook does four critical things:
I built this template from the exact same master document I charge private clients $4,500+ to customize. It contains 38 pages and more than 50 policies, including:
| Category | Key Policies Included |
|---|---|
| Employment Basics | At-Will Employment, Equal Employment Opportunity, Anti-Harassment (including 2024 ILR “respectful workplace” language), ADA & Reasonable Accommodation |
| HIPAA & Patient Privacy | Privacy Officer Designation, Confidentiality Agreement, Social Media & Patient Photos, Breach Notification Procedures, Sanctions for Violations |
| OSHA & Safety | Bloodborne Pathogens Exposure Control, Infection Prevention, Workplace Violence, Emergency Action Plan |
| Compensation & Payroll | FLSA Overtime Rules for Medical Staff, Timekeeping Requirements, Pay Transparency (where required), Meal & Rest Breaks by state |
| Leave & Time Off | FMLA (including 2023 DOL updates), State Paid Sick Leave, Bereavement, Jury Duty, Military Leave |
| Conduct & Performance | Dress Code/Scrubs Policy, Cell Phone & Personal Device Use, Fraternization, Moonlighting, Progressive Discipline |
| Technology & Security | EHR Access, Password Protection, Remote Access, Bring Your Own Device (BYOD) |
| Termination & Separation | Voluntary & Involuntary Termination, Final Paycheck Rules (state-specific timing), COBRA Notices |
Every January and July I personally update the master file with new federal and state requirements. Recent 2024–2025 additions include:
Sources: IRS.gov (independent contractor guidance), U.S. Department of Labor Wage and Hour Division, EEOC, OSHA, and HHS Office for Civil Rights.
Follow these exact steps I give my own clients:
Never simply copy-paste a handbook and forget about it. I’ve seen $125,000 settlements because a 2019 handbook still promised “unlimited PTO” that the practice later stopped offering. Update and re-issue at least every 24 months.

Do I really need a separate handbook just for a medical office?
Yes. Generic templates miss HIPAA sanctions, controlled substance handling, and clinical dress code rules that courts and regulators expect medical employers to address in writing.
Is this template enough, or do I still need a lawyer?
This is the same starting point I use with paying clients. For a 5–25 employee practice with no union or prior litigation, most are comfortable using it as-is after minor edits. If you have collective bargaining, prior DOL audits, or multi-state locations, schedule a paid review.
Can I add a non-compete or arbitration clause?
Yes – placeholder sections are included on pages 34–35 with bracketed instructions.
