The Residential Lead-Based Paint Hazard Reduction Act of 1992 (42 U.S.C. § 4852d), commonly called “Title X,” requires sellers and landlords of most pre-1978 housing to:
The rule applies to virtually all housing built before 1978 unless officially certified lead-free.
Many states (including New York, Massachusetts, Wisconsin, New Jersey, and others) have additional notification or certification requirements on top of the federal rule. Using only the generic federal form in those states can expose you to liability.

New York has one of the strictest regimes in the country. In addition to the federal form, landlords in NYC and statewide must:

Massachusetts requires the state-specific “Massachusetts Lead Law Notification” form and, for properties with a child under 6, a Letter of Compliance or Interim Control must be attached.

California follows the federal form but additionally requires the pamphlet in Spanish if negotiated in Spanish, and agents must ensure compliance (Civil Code § 1102.6 for sales).
The federal template above is sufficient, but many California agents prefer the CAR Form LBP.
Wisconsin requires its own state disclosure form (Wis. Stat. § 709.03) in addition to the federal one for sales, and recommends it for rentals.
