Step-by-step instructions I give every client:

1. Fill in your full legal name, address, and agent information.
2. Initial the powers you wish to grant (or cross out ones you don’t).
3. Sign the Notice page and the main signature page in front of two disinterested witnesses and a Pennsylvania notary simultaneously.
4. Have every agent and successor agent sign the Agent Acknowledgement form in front of a notary (can be different notary).
5. Make at least five copies: original for agent, one for each successor, one for your safe deposit box, one for your attorney or executor.

Philadelphia County recording tip: If you ever want to record the POA for real estate transactions, the Philadelphia Department of Records now requires the Agent Acknowledgement to be attached as Exhibit A.
