Frost Bank (routing 114000093) provides their official form inside online banking under “Account Services → Direct Deposit.” If you can’t log in, use the generic template above and attach a voided Frost check – employers almost always accept it.
Green Dot does not publish a standalone form. Instead, log into your account → “Direct Deposit” → “Get Form” generates a pre-filled letter. Many users simply submit the generic template with routing 124303120 (The Bancorp Bank) or 096017780 (Bonneville Bank, Walmart MoneyCard).

Allied Universal uses UltiPro/UKG. Employees log into the employee portal and set up direct deposit electronically – no paper form needed. If required, the generic template works.
After the 2023 merger, use U.S. Bank’s online enrollment or routing 122000496 (California) with the generic template.
Can I use a letter for direct deposit instead of a form?
Yes. Many employers accept a signed letter containing the same information shown in the generic template above.
Do I need to attach a voided check in 2025?
Only if your employer or payroll provider specifically requests it. The signed authorization form alone is usually sufficient.
